1. Information About Principal Investigators/Project Directors (NSF Form 1225)

The National Science Foundation is committed to providing equal opportunities for participation in its programs and promoting the full use of the Nation's research resources. To aid in meeting these objectives, NSF requests information on the gender, race, ethnicity, and disability status of individuals named as Principal Investigators or Co-Principal Investigators on proposals and awards. Except for the required information about current or previous Federal research support, submission of the information on the form is voluntary, and individuals who do not wish to provide the personal information should check the box provided for that purpose. The forms and the information they contain are accessible by NSF personnel only.

By signing Page 2 of the Cover Sheet for Proposal to the National Science Foundation, proposers are providing the required certifications. See Section II. C.1 for further information regarding certifications.

3. List of Suggested Reviewers or Reviewers Not To Include (optional)

Proposers may include, in a cover letter or separate sheet of paper (so that it can be easily removed), a list of suggested reviewers that the proposers believe are especially well qualified to review the proposal. Proposers may also designate persons they would prefer not review the proposal, indicating a reason why. These lists are optional and may be omitted. The NSF Program Officer handling the proposal decides whether to follow the suggestions, and may contact the proposer for further information.
